Update on Rheumatology appt. today Options

[size=7]Hi everyone, what a day!! Appt was for 10 and got in to see him at ll.40. He believes that I do have Sjogren's with the RA, wasnt clear on which is primary. He has made me an appt at the Sjogren's clinic and I have to wait for the letter. He asked me to mark on paper where the level of my RA is right now from 1 to 10, I said its about 8, but only since being back on pred. I have to gradualy decrease the pred from 15mg to 10mg after another 10 days then two weeks after that to 7.5mg for 4 weeks. I have another appt with him in 4 weeks time.

He feels that it could be the sulfa causing problems re rash, and was not sure whether I should try it again, I did say I would rather not, so he is looking at Leflunomide, he gave me a leaflet to read.

I went through my list with him, and asked if he really thought we could get it under control, he said that he thinks we should be able to, and thats why he wants to see me monthly until things do become under control. I am to have bloods done every 2 weeks again. He said that he thinks I had a flare last week due to coming off pred too quickly, even though I told him that at 5mg I am in alot of pain, and continued to decrease as he recommended at visit in April. He said if I flare like that again i am to call for an earlier appt. I asked about anaemia and he said all bloods are fine. He again looked at my bruising and asked if I have heard anything yet from haemaphillia dept, which I havent. I asked about pain control and he suggested upping the hydroxy to 400mg the mtx to 25mg and the pregabalin for the fribro to 300mg he hopes this will help, I REALLY hope so!! Told him about pins and needles in hands and feet and numbness in feet too.

I really felt he listened properly, and was prepared to understand how ill I have felt lately. Glad you feel you got somewhere today, been concerned for you being in so much pain. I feel better today, got up at lunchtime feeling more like myself no tiredness or aches. Smile Had a fun day in the sunshine was great fun. Hope you feel better soon love from Lorna xx

Glad it went well for you Barbara, and that's good that he's seeing you every month till things are on a more even keel.
I hope all the changes in drugs works well for you.

thats good hes such a sweety very professional and knowledgable to.

u can ask him next time which he feels is the primary one.
ohh yes anaemia would show in full blood count its the hb level.
aww hun alfare u had yes,good he listened so much and answered alot for you to.

good idea bloods every 2 weeks it will hsow him how you are if flaring what body doing terms of new medication very through indeed.

sounds like through appointment wish id see him all time,only seen him once then trainee docs rest time havnt seen consultant sine novmeber last year grr.

u must be exhausted.rest up and il catch up with you sunday see how are.

Hi Barbara

Thanks for posting when you must be exhausted.

We've all been really worried about you lately as you seem to be in such a lot of pain. At least you have a very good, supportive rheummy who seems determined to get to the bottom of things and sort out your meds. It's good to know that your bloods are being checked so frequently too.

Take care, lovely Barbara. Try to relax over the weekend (easier said than done, I know) and I hope and pray that the new tablet strength and doses help greatly.

~Thank you for the replies. Feeling very tired today, but not so much pain and swelling. Had awful night sleep wise, know everyone did, way TOO HOT!!! Mely, it's not right that you don't get to see consultant, I must say that since January I have seen Dr Garrood every time. I'm surprised that you haven't seen him, as with three AI illnesses I would think they need to keep a strict check on things.

Will take it easy this weekend, and hope everyone copes with the heat. Thank you all again for all the support x xSmile

So sorry to hear that you are feeling so poorly at the moment. Do hope things start to improve soon and you are able to get some relief from the pain and the worry. Sounds like you are in good hands with a sympathetic and understanding doctor. Take care Barbara and try to rest when you are able.

Hi Barbara aww h was really through so pleased for you,enjoy the weekend resting up.
yes i saw mr garood last novemebr when all through test were done,and then was booked in for him see me in 4 months but when i got there he wasnt on duty only mr g and saw him wisper only call me in to me if serious problems.

then casue i said want child ohh are hands are tied we can only give u planequil steriods that it you do relaise what your doing,and no explantion what he meant my gp was not impressed at all and has had pick up pieces.

i wasnt happy with this,i hope i see mr garood next time to go over what hes given me how its working or not,and discuss things more.

anyway this post is for you hun.
so enough bout me lol.

hope the extra meds kick in soon for you.

Have you seen a O.T they are very helpfull .By doing a rotine for you .

like if i stick to what the O.T says and rest when she says and be on my feet when she says i never got pain and was so much better

did my own thing pain swelling .
they can show you how to sit propaley to get max rest while you are say out having a coffee in the car
And tell you when to rest .this really really works and i was less tried and less pain and could get though .

She wrote me a plan to stick too working it in with what you do .

Dear Barbara

I'm glad you have been listened to, it seems your consultant will be keeping a close watch on you, which can only be good. I'm sure with the right meds you will begin to feel better. I know what you mean about the heat, as well as putting my feet in cool water every evening, last night I went to bed with a COLD water bottle for my feet!

I hope the pred is making things better.

Thank you everyone for all the replies. Chrissie, I haven't seen an OT since diagnosis in July last year. I did ask about podiatrist and OT when I saw specialist nurse last month and she said she can refer me to podiatrist, but still havent heard anything. How else can I get this done? She said that I can refer myself for OT but didnt explain how. You are right, of course, seeing the OT would be a great help, I should have done this ages ago, but haven't got the information on how to. Had a restful weekend, been lovely, and feet not so painful or swollen, also other joints not so bad. Back to normal tomorrow!! Will be getting organized for tomorrow soon.Smile

You should be able to get OT contact number from gp, hospital - or try their websites too - might be on there.
In our area, can ring direct for an appointment,
Im sure its not the same everywhere but should be much easier to get this information especially from specialist nurse - they should know and share info better,
P'raps make some enquiries tomorrow, do let us know how u get on xx
OT should then be able to give contacts for podiatry,

Hi Barbara,

I am so pleased to hear, that you had a proper talk about all your problems. It makes you feel much better, when somebody listens and weighs up the different options. RA is such a complex illness. The worst is, when we are not taken seriously and fobbed off.

Like you I am also going through a very difficult patch at the moment with ESR at 112 and CRP at 128. A depo the previous week did not make any difference, so I phoned up the hospital on the monday. The rheummy told me to up the predniselone from 4mg to 10 mg for 7 days. It seems to have helped a little bit with the help of co-codamol every 4 hours. Today I am seing the rheumy again.

I do hope, you had a restful week-end. It was a bit cooler last night and this week it should not be so hot. I have really found this weather very tough.

Hi Barbara,

Just seen your post and really sorry to hear that the old RA has been giving you so much grief. Let's hope, that now that your rheumy wants to see you monthly that s/he will be able to help get the illness under control.

Regarding Chrissie's suggestion of seeing an OT, I'm surprised that you have not been referred yet. If the RA nurse can't refer you, then your GP should be able to do this for you. Last year I was referred by my GP and received useful info on pacing to conserve energy, plus I received a lot of useful gadgets for the kitchen, help with my self care (bath board), walking aids and splints for my wrists. To my knowledge, there are two OT services; one run by the NHS an one by social services. Your GP should be familiar with these services and should refer you to the appropriate community service or local hospital for an assessment of needs.
